The piston shown in the video isn't "big" in their world. Some of their ships engines have doors on the crankcase---so you can go in and check the place out
They also have a threaded hole on the top of the piston, which in operation has a plug in it. The purpose of it is for maintenance. They take the plug out, install a lifting eye and pull the piston and rod out with a crane.
